Yes. You will need a current bill or statement from your provider as well as an Explanation of Benefit form from Empire Blue Cross Blue Shield or MetLife. Also, the SRP cannot accept co-payments from Medco Health or Empire BlueCross BlueShield. The SRP is not a reimbursement plan similar to the Security Benefit Fund. If you pay your medical (dental) claims or tuition bills you will not be allowed to take monies from the SRP as a hardship withdrawal.

Completing a change of address form and submitting it to the Fund Office, will change your address with the Fund Office, Empire BlueCross BlueShield, Medco and MetLife. You must notify the union separately so that your address will change with them as well.

You can log onto New York unemployment’s website, enter your Social Security number and the PIN number that you assigned the debit card and printout a transaction history of all payments that were made to you by unemployment.
