Appeal Denied Claims
Your request for a review of a denied claim should be
made in writing and sent, within 60 days of the receipt of
the denial, to:

You may examine plan records relating to your claim or
submit comments on your claim in person or through a
representative.
The Administrator will present your request for review to
the appropriate provider of services, and/or the Trustees.
The provider or the Trustees will respond to your appeal
within 60 days, unless special circumstances make it
necessary for them to take an additional 60 days to review
your request. You will be notified of the need for an
additional 60 days before the end of the initial 60 day
period.
If you feel that legal action concerning the Welfare Fund
is necessary, legal process may be served upon the
Administrator, or upon one or more of the Trustees at the
addresses shown in the front of this booklet.
