Metal Trades Branch - Welfare Fund

To All Participants

We are pleased to present you with this Benefit Booklet, which describes in detail the benefits to which you and your dependents may be entitled to as an eligible participant of the Metal Trades Branch Local 638 Welfare Fund.

While the booklet contains a description of the benefits provided by the Fund, the actual payment of these benefits will be predicated upon the terms and provisions of the group contracts between the Board of Trustees and the "providers" of these benefits. Any questions concerning a claim or the benefits received should be directed to the Fund Office. Furthermore, in the event a claim is denied, you have the right to appeal. Specific instructions as to how and where to file an appeal are set forth in Section 17 of this booklet.

Should you have any questions about your eligibility, your classification under the plan, or any benefit, please call the Fund Office. The Fund personnel are the only persons authorized by the Trustees to answer your questions.

Finally, we urge you to review this booklet carefully so that you may be fully aware of the benefits provided and the steps that must be taken in order to receive these benefits. If you have any questions that are not answered by the material contained in this booklet, we encourage you to contact the Fund Office or any of the Trustees.
